Care teams are all different and might have different workflows for how they address patients based on their use case, internal team structure and individual preferences. While Prosper Care has some recommended workflows for particular use cases, we wanted to empower teams to be able to build their own. Let's find out how to build your own Views.
1. When one logs into Prosper Care, a default Monitoring view is presented.
2. To add or remove columns of patient attributes, click the vertical dot menu.
3. Click "Select columns"
4. A list of patient attributes to add as columns will be presented. These attributes will be added as columns if checked, and removed if not checked.
5. Pick a column and choose the option(s) or type the selected value for that column and click "Filter".
6. If desired, click on the columns to sort in ascending or descending order
7. A 'Save View' button will appear. Click the icon to save the view
8. Enter a short name and description (which will appear on mouseover.) Add optional highlighting
9. Click "Add view"
10. The card will now be presented in the top of your Population Views with the number of patients matching that criteria.
11. If you place the mouse over the view name, you will see the view description.
12. If you want to delete a view, click the view's name and then the delete button.
Try not to cram every field into one view. Think about the questions you want to address in a view for your workflow and build it for that. Which of my patients need to be addressed first (e.g. who had recent concerning values)? Which patients do I need to call for onboarding (e.g. when were recent device delivery dates?) Which patients should I call to make sure they are adherent? (e.g. When were higher acuity patients with CHF last active?)
