Once in this tab, the user can navigate between any of the tabs and document notes.
At the top, important patient identifying information, including the patient’s name, ID, date of birth, phone, assigned site, date since last onboarded, and city/state. This information can be edited in the Demographics tab.
Results Tab
The Results tab displays Result Cards, which include data such as device measurements, scores, and other values captured by remote monitoring and patient-reported data.
The Results Cards provide:
- An overview of the latest results captured
- A quick view of things that might require attention
- Streamlined navigation to the details associated with the measurements.
Definitions:
- Blood Glucose, Blood Pressure, Heart Rate, and Weight: A recording of the
most recent physiologic data recorded in Prosper and includes the
date it was recorded. - Days with measurements: This applies to RPM patients and is a count of
how many days during the patient’s 30-day service period that they
have transmitted data. - Consultation: This applies to RPM patients and is a count of how many
minutes were spent with or on behalf of the patient during a calendar
month. - Completed content tasks: In instances where patients complete questionnaires
or surveys, educational content, etc., data populates the Completed content
Tasks card in the patient Results Tab. The table view is available by
clicking on the card.
Clicking on a Result Card will display:
- longitudinal graph of the result values over time
- a results table showing additional details and actions associated with each result value.
Result Cards are 'Grey' when the results are within acceptable ranges and 'Red' when results are out of range.
