Prosper Care: Demographics Tab

The demographics tab contains the patient’s account information and is where the
user edits and adds patient detail.

Tags:
These are descriptors that a user can assign to a patient (e.g., visually impaired,
hearing impaired, limited cell phone minutes, etc.). These descriptors or ‘Tags’ can
be used to communicate within the team and are visible as a column on patient
lists, allowing for sorting, analyzing, etc. Each patient can be assigned more than
one tag and each user can create tags.
NOTE: For internal consistency, it is suggested that teams standardize tags, including
their definitions and use cases.

Address
Shipping address information is required when the patient’s device(s) will be
shipped. The address may be different from their home address.

Contact
This information is necessary and used to send the patient a Short Message Service
(SMS), email, and/or voice messages to encourage them to use their device to
obtain a physiologic measurement, complete a survey, or participate in educational
activities. At least one (1) of these options is strongly recommended, mobile phone
preferred.
NOTE: Checking the “Mute all notifications until next device usage” will temporarily stop
sending reminder nudges (SMS or voice) to the patient until they record a measurement.

Billing Information

This information is used  when services are eligible for insurance reimbursement (e.g., RPM).

This information is used when services are eligible for insurance reimbursement (e.g., RPM) and when a user desires to keep track of the patient's insurance.
