Notes can be used by any user and the user should follow internal policies and procedures for note use (e.g., document in EMR or Prosper Care). The notes feature is generally used for RPM reimbursement requirements (e.g., CPT codes 99457 and 99458) to keep a record of patient communication and data review, including the number of minutes each activity took. Notes can be used to record additional information about a patient that is not captured in other locations within Prosper Care.
Creating Notes
To create a new note, the user makes the following selections:
1. Note type
2. Check the box to indicate that a note reflects a conversation with the patient
and/or their caregiver (interactive communication is required to bill RPM).
3. Choose a note template or free text the note.
4. Timestamp (date/time)
5. Time spent (e.g., for RPM)
Once the user fills in these fields, they will click ‘Save’ to attach it to the patient’s record. These activities are always visible within Prosper Care and can be exported (e.g., in the case of RPM where they are used as supportive documentation by physicians, advanced practice registered nurses, or physician assistants who are billing RPM services.)
There are icons next to each note in Prosper Care and each icon provides visual information about the note. Users may edit their notes for up to 24 hours after the note is written.
